The Targeted Case Management (TCM) Program assists individuals with severe and persistent mental illness, referred by Milwaukee County, in achieving the highest possible level of independent living within the community.

City Outreach’s strengths-based Case Managers provide comprehensive assessment, individualized support, and ongoing monitoring. They coordinate services across medical, psychiatric, AODA, social service, faith-based, employment/vocational, and peer support systems, ensuring that each consumer is connected to the resources needed for stability and growth.

The TCM Program helps reduce the likelihood of re-hospitalization and acute mental health crises by addressing critical areas of need. Consumers are supported in securing safe housing, accessing legal services, obtaining medical and behavioral health care, managing finances, pursuing employment and training, navigating benefits, and strengthening daily living skills. Case Managers also assist with building social networks, enhancing natural supports, and developing effective crisis prevention and intervention strategies.

TCM services are authorized exclusively by the Community Access to Recovery Services (CARS) program within the Behavioral Health Division of Milwaukee County. For more information or to make a referral, contact CARS at (414) 257-8085.
